Qualifications

• Bachelor's degree in a human services field plus two years of transferable case management experience.
• Knowledge of the criminal justice system, human behavior, and social work skills.
• Ability to maintain confidentiality.
• Strong organizational skills with detailed records.
• Ability to travel and work flexible hours.

Responsibilities

• Complete a needs assessment and develop an individualized service plan for each participant upon entry.
• Maintain detailed participant files and ensure records comply with policies.
• Conduct intake and evaluate job readiness, incorporating goals into the IDP.
• Develop the IDP with results, training goals, and milestones every 6 months.
• Provide counseling and ongoing one-on-one coaching on employment readiness.
• Support enrollment in vocational training and education pathways.
• Coordinate job-search efforts and employer engagement strategies.
• Provide job-retention coaching for up to 12 months post-employment.
