Responsibilities

  • Complete a needs assessment and develop an individualized service plan for each participant upon entry.
  • Maintain detailed participant files, documenting services, progress, and outcomes, and ensure records are complete and accurate per program policies and procedures.
  • Conduct intake and orientation for new participants; evaluate job readiness and incorporate employment and training goals into the IDP.
  • Develop the IDP, including assessment results, training/credential goals, career pathway, and milestones, upon entry and every 6 months.
  • Conduct regular file reviews to ensure documentation is complete and current.
  • Provide counseling and support on personal, family, financial, employment, behavioral, and educational matters.
  • Provide ongoing one-on-one coaching focused on employment readiness and retention.
  • Support enrollment in vocational training, certification programs, and education pathways.
  • Support participants' job search efforts and job-search strategy.
  • Provide job retention coaching for up to 12 months post-employment.
  • Identify and address barriers including transportation, housing, childcare, and legal obligations.
  • Coordinate referrals to community resources and partner agencies.

JOB SUMMARY

The RISE Case Manager provides individualized career coaching and case management services to justice-involved young adults ages 18-24 returning to the community, served by Volunteers of America Southeast Louisiana. This role is responsible for supporting participants in achieving employment, education, and financial stability goals through an integrated service delivery approach. The case manager addresses barriers such as housing instability, transportation, limited work history, and legal challenges while facilitating access to workforce training, employment opportunities, and supportive services throughout the Louisiana service area of Jefferson, Plaquemines, Orleans, and St. Tammany parishes.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Complete a needs assessment and develop an individualized service plan for each participant upon entry
  • Maintain detailed participant files, documenting services, progress, and outcomes, and ensure records are complete and accurate per program policies and procedures
  • Conduct intake and orientation for new participants; evaluate job readiness and incorporate employment and training goals into the Individual Development Plan (IDP)
  • Develop the Individual Development Plan (IDP), including assessment results, training and credential goals, career pathway, and employment milestones, upon entry and every 6 months
  • Conduct regular file reviews to ensure documentation is complete and current
  • Provide counseling and support to participants on personal, family, financial, employment, behavioral, and educational matters
  • Provide ongoing one-on-one coaching focused on employment readiness and retention
  • Reinforce job readiness skills (resume development, interview preparation, workplace soft skills) in coordination with the RISE Case Manager
  • Support enrollment in vocational training, certification programs, and education pathways
  • Support participants' job search efforts in coordination with the RISE Case Manager’s individualized job-search strategy
  • Provide job retention coaching for up to 12 months post-employment
  • Identify and address barriers including transportation, housing, childcare, and legal obligations
  • Coordinate referrals to community resources and partner agencies
  • Support participants in obtaining identification, work documentation, and expungement resources where applicable
  • Coordinate with the RISE Case Manager on employer engagement opportunities for participants nearing job readiness
  • Coordinate interviews, job fairs, and hiring events with the RISE Case Manager
  • Coordinate with RISE staff and community partners on participant service needs and case planning
  • Perform other duties as assigned consistent with the RISE program.

ADDITIONAL DUTIES AND RESPONSIBILITIES (Non-essential)

  • Oversees and/or participates in the printing, binding and assembly of training materials as well as the transportation of materials to various training faculties
  • Coordinates acquisition, transportation, assembly and use of training equipment including computers, digital projectors, screens, sound systems and other audiovisual equipment necessary for supporting training programs and initiatives
  • Adhere to the designated dress code as stipulated for the specific role.

SUPERVISORY DUTIES:

  • None

KEY CONTACTS

Maintains critical relationships with the following:

  • Maintains frequent contact with Department of Corrections pre-release programs and resources
  • Networks with community-based organizations and criminal justice practitioners willing to work with justice-involved individuals
  • Maintains professional relationships with RISE program staff, including the RISE Case Manager

KNOWLEDGE, SKILLS AND ABILITIES:

This position requires knowledge, skills and abilities equivalent to:

  • Bachelor's degree in a human services field plus two years of transferable case management experience
  • Demonstrated knowledge of the criminal justice system, human behavior, and social work skills
  • Ability to work with individuals affected by criminal justice involvement and related personal challenges
  • Knowledgeable of community resources; understanding of the local employment market and available education, training, and vocational resources
  • Ability to maintain confidentiality
  • Strong organizational skills and ability to maintain detailed records
  • Ability to work independently with little supervision
  • Ability to recognize problems and take corrective action per agency and program policy
  • Ability to work flexible hours; ability to travel

Travels to other locations as appropriate in the performance of duties and responsibilities.

Must have:

  • Current driver’s license issued by state of residence
  • Current vehicle insurance
  • Current vehicle registration in state of residence
  • Satisfactory Motor Vehicle Report
  • Must be able to pass a criminal history investigation.

PHYSICAL REQUIREMENTS:

Work is primarily performed in a normal office environment generally while seated at a desk. Dexterity is required to performing work with personal computer keyboard, calculator and other office equipment
