Job summary

Daybreak Inc. in Dayton, Ohio, seeks an Outreach Specialist to connect youth ages 18–24 experiencing homelessness with shelter and housing resources.

You will perform face-to-face triage in shelters, in community settings, and through street outreach to engage unsheltered youth and link them to day programs, employment, healthcare, and personal development opportunities. The role requires strong trauma-informed practices, motivational interviewing, and collaboration with local partners.

Qualifications

• High School diploma or GED with five years of social services experience (or related field).
• Associate’s degree in social work, psychology, criminal justice, or related field with two years of relevant experience.

Responsibilities

• Conduct street outreach to identify and engage unsheltered youth (ages 18–24).
• Provide face-to-face triage and crisis intervention in shelters and community settings.
• Back-up for the Drop-In Center to ensure a safe, supportive space for youth.
• Build trusting relationships with youth using trauma-informed practices.
• Complete intakes, assessments, and HMIS documentation accurately and timely.
• Collaborate with housing, employment, clinical, and case management teams and external partners.
• Maintain confidential client records in CareLogic and HMIS.
• Attend staff meetings and case reviews to ensure coordinated services.
• Represent Daybreak at community events and strengthen referral networks.
