Thresholds in Chicago seeks a Community Support Specialist for Emerging Adults to provide community-based mental health support and care planning for young adults with serious mental health challenges. You will connect clients with resources, remove barriers to care, and help them navigate systems to achieve goals.

Qualifications

• Passion for mental health advocacy with the youth and young adult population.
• Patience and willingness to meet young people where they’re at
• Strong time management and organization
• To be at least 21 years old
• A valid driver’s license, current car insurance, and daily access to your own car
• Willingness to provide transportation to clients

Responsibilities

• Provide mental health support to young adults in the community.
• Conduct care planning and connect clients with resources; assist with navigating systems.
• Drive clients to appointments, benefits offices, and grocery store.
• Facilitate activities and programs contributing to development.
• Adapt to client schedules, needs, and treatment plans.

Education

High school diploma or GED

Bachelor’s degree in Psychology, Social Work, or related field

Bachelor’s degree in an unrelated field and 2 years of supervised clinical experience

Master’s degree in Psychology, Social Work, or related field
