Job summary

Crawford County Mental Health Center (CCMHC) is seeking a Direct Support Specialist to provide goal directed, solution-focused interventions for youth with SED. You will work in community settings, guided by clinicians, to help clients access resources, implement treatment tasks, and advocate for needed services.

Qualifications

• Be able to communicate effectively in spoken and written English.
• Possess a valid driver’s license.
• Operate office equipment.
• Demonstrate ability to work effectively with youth with mental illness.
• Ability to operate effectively within a wide variety of human service situations.

Responsibilities

• Assist in assessment of client needs.
• Assist clients in accessing community-based resources including transportation and access to health-related providers.
• Assist clients in implementing tasks related to treatment plan goals.
• Collaborate with community partners to follow treatment plan goals and secure resources.
• Advocate and empower clients to obtain necessary services and benefits.
• Provide information and support to family members as appropriate.
• Assist with crisis management as needed.
• Complete all required documentation in the EMR in a timely manner.
