Job summary

Vital Health in Northwest Ohio seeks a licensed clinician to provide mental health and substance use disorder treatment in outpatient programs (PHP, IOP, Day Treatment). You will complete diagnostic assessments, develop treatment plans, and deliver individual and group counseling.

Responsibilities include facilitating PHP/IOP/Day Treatment groups, coordinating with a multidisciplinary team, conducting home visits when needed, and ensuring timely documentation in the EHR; schedule may include

Qualifications

• Licensed clinician with evidence of practice in mental health and substance use treatment.
• Candidate must hold Ohio licensure or license-eligible credentials.
• Experience in outpatient settings and EHR documentation is expected.

Vital Health provides integrated primary care, behavioral health, and substance use treatment for adults and children in Northwest Ohio. We are seeking a licensed clinician to provide mental health and substance use disorder treatment in our outpatient programs (PHP, IOP, Day Treatment).

Requirements

• Active Ohio LSW, LISW, LPC, or LPCC (required)
• Bachelor's degree required (Master's preferred)
• Experience in substance use and mental health treatment
• Knowledge of withdrawal stages and crisis intervention
• Strong understanding of confidentiality, patient rights, and professional ethics
• Ability to work collaboratively in a multidisciplinary team
