Maryville Academy in Illinois is seeking a dedicated Care Coordinator to support youth and families across behavioral health, juvenile justice, education, housing, and community services.

This role emphasizes trauma-informed, youth-centered approaches, cross-disciplinary collaboration, careful documentation, and active participation in court- and school-related meetings, with a competitive salary and benefits.

Formación

• Bachelor’s degree in social work or related field.
• Two years of experience with adolescents and families preferred.
• Knowledge of juvenile justice, probation, and child welfare.
• Proficiency with MS Office and EHR systems.

Responsabilidades

• Care coordination including comprehensive assessments and service plans.
• Engage youth and families using trauma-informed, youth-centered approaches.
• Coordinate with therapists, schools, and providers to maintain care.
• Support education, employment services, and school transitions.
• Maintain timely case records and required documentation.
• Develop community partnerships and outreach to referrals.

Master’s degree in social work, counseling, psychology, human services, or related field (preferred)
