Job summary

IMPOWER is seeking a Supportive Housing Case Manager to provide individualized case management and housing support to young adults experiencing homelessness in Longwood, FL. This role manages a caseload of 15–22 participants in Rapid Rehousing (RRH) and Permanent Supportive Housing (PSH) programs.

The position requires using Housing First, trauma-informed care, motivational interviewing, and strengths-based approaches; conducting assessments and housing stability plans; assisting with housing

Qualifications

• Associate degree in social services or related field; bachelor's preferred.
• At least two years of case management experience, preferably with youth.
• Knowledge of homelessness, housing programs, and supportive services preferred.
• Ability to work independently in the community and build relationships.

Responsibilities

• Support young adults in housing, employment, education, and other goals.
• Provide housing-first, trauma-informed, motivational interviewing, and strengths-based services.
• Complete assessments and develop individualized housing stability plans.
• Assist with housing searches, placements, and coordination with landlords.
• Conduct twice-monthly home visits and monitor progress.
• Connect participants with community resources and supports.
• Maintain case notes and documentation in HMIS.
• Collaborate with landlords and partners to support stability.
