Job summary

IMPOWER in Longwood, FL is seeking a Supportive Housing Case Manager to provide individualized case management and housing support for young adults experiencing homelessness. The role manages a caseload of 15–22 participants in RRH and PSH programs, working with landlords, service providers, and partners to promote housing stability.

The position requires an Associate degree (Bachelor’s preferred) and at least two years in case management.

Qualifications

• Associate degree in social services or related field (bachelor’s degree preferred).
• At least two years of case management experience, preferably with youth/young adults.
• Knowledge of homelessness, housing programs, and community resources preferred.

Responsibilities

• Assist with housing searches, placements, move-ins, utilities, and coordination with landlords.
• Provide Housing First, Trauma-Informed Care, Motivational Interviewing, and strengths-based approaches.
• Complete assessments and develop individualized housing stability plans.
• Maintain HMIS case notes and documentation.
• Collaborate with landlords and partners to support housing stability.
