Job summary

Youth Haven Services is seeking a compassionate Qualified Professional to deliver Intensive In-Home services to children, adolescents, and families in Forsyth County, NC. The role includes clinical oversight, case management, and skill-building, working with licensed therapists and partners to achieve goals.

Work is home- and community-based, with a W-2 arrangement and compensation based on billable contacts.

Qualifications

• Bachelor's degree in a Human Services-related field is required.
• Two years of documented experience with children, adolescents, and families in behavioral or mental health services.
• Valid North Carolina driver's license and reliable transportation.
• Ability to travel throughout Forsyth County to provide home and community services.
• Flexible schedule including afternoon and evening appointments.
• Strong engagement and relationship-building with youth and families.
• Excellent written, verbal, and interpersonal communication skills.
• Knowledge of NC Intensive In-Home service definitions and documentation requirements.
• Proficiency with electronic health records and Microsoft Office applications.

Responsibilities

• Complete client intakes, assessments, and Person-Centered Plans (PCPs).
• Submit initial authorizations and reauthorizations for Intensive In-Home services.
• Coordinate care with therapists, families, schools, physicians, and other providers.
• Meet with clients and families in homes, schools, and community settings for case management and skill-building.
• Monitor client progress and assist families in achieving treatment goals identified in the PCP.
• Maintain accurate, timely, and compliant clinical documentation, including progress notes and service logs.
• Complete NC TOPPS assessments, Service Authorization Requests (SARs), discharge documentation, and other state-required documentation.
• Maintain assigned caseloads and ensure authorizations remain current.
• Assist families in accessing community resources and supports.
• Participate in Child and Family Team (CFT) meetings and other treatment planning meetings.
• Perform other duties as assigned to support the Intensive In-Home program.

Services are delivered primarily in the client's home and community and are guided by each individual's Person-Centered Plan.
