Job summary

Four Seasons seeks a Child & Adolescent Grief Coordinator to deliver high-quality, compassionate grief care to children, teens, and their families admitted from hospice or palliative care. You will conduct in-person counseling, phone support, and Compass program activities to support coping and healing.

Reporting to the Director of Patient & Family Services, you will develop individualized care plans, document notes, coordinate school-based groups, and collaborate across teams while

Qualifications

• Bachelor’s degree in Social Work, counseling or related fields; Master’s degree preferred.
• 2 years’ work experience in client-care setting with children and adolescents; counseling for grief/loss preferred.
• Licensure/certification for designated educational background; valid driver’s license and automobile insurance.

Responsibilities

• Provide grief counseling to children and teens; in-person and phone support.
• Develop individualized care plans addressing psychosocial needs and goals.
• Document concise clinical notes detailing plan of care.
• Collaborate with team members; coordinate school-based grief support groups.
• Introduce and facilitate coping techniques tailored to individual and family needs.
• Maintain confidentiality of client and family information.
• Discharge planning for Compass Bereavement Care clients.
