Job summary

Catholic Charities of Baltimore, Villa Maria Behavioral Health seeks a Family Care Coordinator to work with a child and family facing impairments in school, family, or community functioning. The FCC develops a family-driven care plan and supports progress via community resources and motivational interviewing.

Typical schedule is Monday–Friday, 10 AM to 6 PM. The clinic offers mental health services including counseling, evaluation, skill development, addiction treatment, and medication

Qualifications

• High School/GED and 2 years of work experience with emotionally disturbed children and families.
• Bachelor’s degree in social work, psychology, or related field plus 1 year of work experience with emotionally disturbed children and families.

Responsibilities

• Conduct a family needs assessment and develop a care plan/IRP with the family.
• Use motivational interviewing with youth and family to pursue goals.
• Coordinate meetings and communicate with outside providers, including schools.
• Help families access community supports for basic needs, medical, and transportation.
• Maintain written records of work, including assessments and meetings; document in EMR.
