Job summary

Integrated Services for Behavioral Health seeks an Engagement Care Coordinator to assist youth hospitalized with behavioral health conditions. This role focuses on supporting families during the discharge process and educating them about ongoing care options.

The ideal candidate will have experience in behavioral health, excellent organizational skills, and be culturally responsive. A comprehensive benefits package, including medical and 401K, along with a supportive work environment, is offered.

Qualifications

• 3 years experience with a high school diploma; 2 years with an associate degree; or 1 year with a master's degree.
• Knowledge of Hi-Fidelity Wraparound preferred.
• Experience with autism spectrum disorders and developmental disabilities.
• Have qualifications to work with families facing serious emotional disturbance.

Responsibilities

• Assist in immediate care coordination needs of youth hospitalized due to behavioral health conditions.
• Educate families about the OhioRISE program and provide referrals for follow-up care.
• Engage and build positive relationships with families and youth.
• Develop partnerships with community resources.
