Paylocity is seeking a Community Case Manager to provide high-quality individual and family support for adolescents and their families across office, community, and in-home settings. The role supports clinical decision-making and requires thorough documentation of assessments, goals, and treatment activities.

Responsibilities include crisis planning, referrals, advocacy with external agencies, and maintaining required caseloads.

Qualifications

• Bachelor’s degree in social work or related field preferred.
• 2 years of related experience or training or equivalent combination.
• Strong verbal and written communication skills.
• Valid driver’s license with good driving record.
• Computer literacy and data entry skills.
• Ability to relate to and show empathy for youth.

Responsibilities

• Provide individual and family support to youth and families.
• Assess client and family needs to develop individualized goals and treatment plans.
• Deliver community-based and in-home interventions.
• Document initial and ongoing assessments and treatment plans according to standards.
• Engage in crisis planning with youth and families.
• Refer and link to community services as needed.
• Advocate for youth with external agencies.
• Maintain caseload and meet productivity standards.
• Participate in trainings and supervision meetings.
