Qualifications

• Bachelor’s degree required; state-regulated acceptable fields include Psychology, Sociology, Social Work, Nursing, Counseling, Public Health, Education and related disciplines.
• Criminal background check and drug screening required.

Responsibilities

• Complete written needs assessment.
• Assist with treatment plan development based on assessment data.
• Monitor progress and ensure services accessed as per plan.
• Provide care consistent with professional standards and ethics.
• Maintain accuracy and completeness of client files.
• Deliver services within agency time requirements.
• Document services in client treatment plans.
• Advocate for consumers when needed.
• Avoid duties outside the position’s scope.
• Respond promptly to emergencies and coordinate services.
• Collaborate effectively with a multidisciplinary team and independently when required.
• Other duties may be assigned.
