Job summary

Integrated Services for Behavioral Health (ISBH) seeks an Engagement Care Coordinator to support youth in OhioRISE, coordinating hospital discharge, and educating families about available resources. You will participate in meetings, develop care and crisis plans, and ensure smooth handoffs to ongoing care.

The role emphasizes engagement with families, cultural humility, and collaboration with community partners to meet diverse needs in Pickaway County and surrounding areas.

Qualifications

• Experience in coordinating services for children and families
• Knowledge of behavioral health and public sector human services
• Hi-Fidelity Wraparound experience preferred (Certification provided)
• Ability to work with diverse families and communities

Responsibilities

• Assist in immediate care coordination needs for youth in OhioRISE
• Support hospital discharge process and family education
• Attend meetings to complete engagement milestones and warm handoffs
• Develop care plans and crisis safety plans with families and team
• Maintain documentation and participate in program evaluation
