Job summary

Integrated Services for Behavioral Health (ISBH) in Pickaway County seeks an Engagement Care Coordinator to support youth enrolled in OhioRISE. You will assist families with discharge planning, educate about the program, and arrange referrals for follow-up care in home, school, and community settings.

You will participate in in-person and virtual meetings to complete care coordination milestones, develop safety plans, assessments, and care plans, and facilitate Child and Family team meetings for

Qualifications

Experience working with children and families in behavioral health or related public services.
Able to manage multiple tasks with strong organizational skills.
Willing to participate in cross-systems care coordination and meetings.

Responsibilities

Assist in immediate care coordination for youth in OhioRISE programs.
Support families with hospital discharge planning and referrals.
Educate families and partners about OhioRISE and services.
Attend in-person and virtual meetings to complete milestones and facilitate Child and Family team meetings.
