Qualifications

• Experience providing services to children and families in behavioral health or related public sector human services.
• Two years of experience in a coordinated supportive services or care coordination role preferred.
• Knowledge or certification in Hi-Fidelity Wraparound preferred (Certification provided at time of employment).
• Valid Driver's License required.
• Able to work effectively with multi-system teams and diverse communities.

Responsibilities

• Identify care coordination needs with families to align with service delivery standards.
• Coordinate family-based services in home, school, and community.
• Ensure services meet safety and treatment needs and are least restrictive.
• Develop partnerships with community resources to support youth and families.
• Maintain documentation and participate in program evaluation and planning.
