Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a dedicated OhioRISE Care Coordinator in Muskingum County, OH. This role involves assembling and guiding a family team, coordinating services at home, school, and in the community to keep families together and reduce hospitalizations.

The ideal candidate has experience in behavioral health or related public services, and can manage deadlines in a fast-paced environment.

Qualifications

• Experience delivering services or support to children and families in behavioral health or related public services.
• Knowledge of coordinating care across home, school, and community settings.
• Ability to bill Medicaid without restrictions and to work with multiple partners.

Responsibilities

• Develop, lead, and facilitate team meetings for the youth and identified team.
• Provide care coordination within the family’s home, community, or preferred location.
• Partner with the family, natural supports, and service providers to identify needs and actions.
• Maintain required documentation to access and maintain OhioRISE benefits.
• Attend mandatory trainings and supervision with travel as required.
• Ensure the youth and families have a voice and choice in all coordinated services.
