A nonprofit organization supporting youth in North Bellmore is seeking a dedicated Case Planner to join the Nassau County PINS Diversion Program. This full-time hybrid role focuses on improving family dynamics and provides essential in-home support for at-risk youth. Candidates should have a relevant degree and experience in casework, with strong communication and tech skills. The position offers competitive pay and a supportive working environment.

Qualificações

• 2+ years of casework experience required.
• Valid NYS Driver's License & access to reliable transportation needed.
• Ability to work flexible hours is necessary.

Responsabilidades

• Carry a caseload of 15-25 families.
• Conduct weekly in-home visits with youth and families.
• Deliver life coaching and case management services.
• Maintain accurate documentation and progress reports.
