Job summary

Hope for Youth is hiring a Case Planner to support at-risk youth and families in Nassau County. This full-time, hybrid role focuses on in-home services under the PINS Diversion Program, helping families strengthen dynamics and reduce out-of-home placements.

You will carry a caseload of 15–25 families, conduct weekly visits, provide life coaching and case management, and participate in school meetings and service planning. Flexible schedule required, own vehicle preferred.

Qualifications

• BSW or Bachelor's in a related field with 2+ years of casework experience, or MSW.
• Strong written, verbal, and tech skills.
• Bilingual (Spanish/English) preferred.
• Valid NYS Driver's License & access to reliable transportation with mileage reimbursement available
• Ability to work a flexible schedule to accommodate families
• Physically able to perform duties (e.g., drive own vehicle, climb stairs, prolonged periods of walking/sitting)

Now Hiring: Case Planner (Social Work, Full-Time, Hybrid/Flexible Schedule)

Nassau County, NY | Field-Office-Remote Role | 35-hour Workweek

Are you passionate about supporting at-risk youth and strengthening families? Hope for Youth is looking for a dedicated and dynamic Case Planner to join our Nassau County PINS Diversion Program!

Our Person in Need of Supervision (PINS) Diversion Program aims to prevent court involvement and out-of-home placements for youth under 18 by providing short-term, family-centered, in-home support throughout Nassau County. We focus on building problem-solving skills and linking families to long-term community resources.

Your Role As a Case Planner

As a Case Planner, you will support youth and families by improving family dynamics, encouraging behavioral change, and reducing the risk of placement outside the home. This role requires flexible hours, including evenings and weekends, and a field-based presence using your own vehicle.
