Job summary

Vanderheyden Hall Inc. is seeking a Youth Peer Advocate to provide youth peer support and training services across home, school, placement, and community settings.

The role collaborates with a triad team including a Family Peer Advocate and a HFW Care Manager, with travel between locations in the Capital District/Troy area. Responsibilities include engaging families, performing youth assessments, and supporting youth through goal attainment and empowerment, using lived experience to foster

Qualifications

• Must be between ages 18 and 30.
• Self-identify as having personal experience with mental health, juvenile justice, special education, or foster care and/or in recovery from substance abuse.
• Completed or able to complete Youth Peer Advocate training and credentialing.
• Must have valid NYS Driver's license and clean MVR.

Responsibilities

• Work closely with triad team members to coordinate services.
• Engage youth and family to establish partnership and access to services.
• Conduct youth assessments to determine strengths and needs for CANS-NY and comprehensive assessment.
• Assist youth in carrying out tasks to meet plan of care goals.
• Provide service components including: Skill Attainment, Coaching, Self-Advocacy, Engagement, and Community Connections.
• Demonstrate leadership and group facilitation skills; participate in supervision.
