Job summary

Recovery Options Made Easy in Jamestown, NY seeks an Interventionist to deliver Home Based Crisis Intervention for children and youth aged 5–20, addressing imminent risk of hospitalization. You’ll provide evidence-based treatment, family support, and linkage to community services to prevent crises from escalating.

The role requires NYS LMHP licensure or eligibility, a small caseload (2–3 families), and collaboration with multidisciplinary teams.

Qualifications

• Licensed LMHP by NYS Education Department or eligibility.
• Experience in community-based mental health or case management preferred.
• Ability to work in home/community settings and coordinate with providers.

Responsibilities

• Provide crisis intervention and stabilization for child/youth and families.
• Conduct screenings and assessments (social determinants, mental health, trauma, safety).
• Coordinate with community services and supports; document in EMR.
• Work rotating 24-hour on-call schedule and maintain confidentiality.
