Job summary

Together for Youth in Rochester, NY seeks a dedicated youth services professional to provide crisis intervention, counseling, and advocacy to diverse youth and families. You will work closely with schools and families to ensure safety and progress.

You will lead groups, coordinate care plans, and document progress, while building partnerships with county agencies and referral sources. A Bachelor's degree in social work or related field is required.

Qualifications

• Bachelor's degree in social work or related field
• MSW preferred
• 1-3 years experience with diverse youth and families
• Flexible schedule including nights and weekends
• Current and valid driver's license

Responsibilities

• Provide steady presence during crisis situations
• Support youth and families throughout the full course of care (assessment, counseling, treatment planning, discharge planning)
• Facilitate psycho-educational and independent living groups (budgeting, job applications, cooking, housing, healthcare, daily living skills)
• Serve as the link between school and home (attend CSE/IEP meetings; advocate for supports)
• Serve in multiple roles (mentor, job coach, educator, advocate)
• Structure home visits as goal-driven time to rebuild trust and communication
• Lead treatment reviews to align the team around a shared plan
• Maintain strong relationships with referring agencies and county caseworkers
• Complete documentation accurately and on time
