Delaware Guidance Services seeks an Engagement Specialist to coordinate care for crisis-affected children and youth, providing proactive outreach and compassionate support. You will reconnect former clients, maintain ongoing engagement, and collaborate with schools, agencies, and providers to ensure seamless services.

The role emphasizes evidence-based Caring Contacts, post-discharge follow-up, and culturally competent care, with office-based work and occasional telecommuting options as needed.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or related field.
• Two years of experience in case management, preferably in healthcare, mental health services, or crisis intervention programs.
• Familiarity with Delaware's behavioral health systems, state regulations, and community resources for children and families.

Responsabilités

• Caring Contacts and Crisis Reconnects: Implement Caring Contacts protocol via phone calls, mailers, and electronic communication to reduce suicide risk and reconnect discharged clients.
• Client Engagement and Follow-Up: Reach out to children and families who decline or disengage to encourage participation and assess ongoing needs.
• System Engagement and Resource Coordination: Coordinate with internal and external providers, ensure follow-up and referrals, and advocate for integrated services.
• Documentation And Reporting: Maintain records of client interactions and prepare regular progress reports.
