Lutheran Social Services of WI & Upper MI is seeking a Crisis Care Coordinator for the REACH Youth Crisis Team in Milwaukee, WI. You will provide trauma-informed, person-centered care coordination to support youth in psychiatric crisis and connect families with essential community resources.

Responsibilities include assessments, individualized service plans, crisis intervention, and collaboration with families and service providers.

Qualifications

• Bachelor's degree in human services or related field.
• Minimum 1 year of mental health service experience.
• Valid driver's license and reliable transportation.
