Job summary

The Guidance Center is seeking a Youth Community Support Specialist to provide CPST to youths and children in Leavenworth and surrounding counties, with services delivered primarily in clients’ homes and community settings. The role requires evenings 1+ day per week after 5 p.m.

and normal business hours Monday to Friday. The position emphasizes strengths-based case management, collaboration with interdisciplinary teams, and flexibility to meet clients where they are.

Qualifications

• Bachelor's degree required; degree in social work, psychology, or human services preferred.
• One year of volunteer or paid experience in the community.

Responsibilities

• Assist clients in determining strengths and needs and defining goals and action plans for community living.
• Participate in treatment planning with clients and Center providers.
• Provide ongoing support for client goal attainment across home, financial, health, relationships, and daily living domains.
• Engage clients and caregivers to maintain engagement and build relationships.
• Help clients and caregivers locate and use resources to meet goals.
• Transport clients to appointments when necessary.
• Facilitate communication among service providers toward defined goals.
• Advocate for clients to help overcome barriers.
• Respond to emergencies and support symptom management.
