Volunteers of America, Dakotas’ Axis180 program seeks a Case Manager to provide comprehensive case management for youth, focusing on housing access, resources, and life skills to foster independence and self-sufficiency.

The role involves intake, ongoing assessment, and coordination of Transition Plans, with collaboration across staff, partners, and community resources to support stable, healthy transitions to independent living.

Qualifications

• Associates degree in social work or related field required.
• One year of related work experience preferred.
• Valid driver’s license and proof of insurance; must be at least 21.

Responsabilités

• Complete intake and discharge activities with youth.
• Meet with youth regularly to address ongoing expectations, progress, or issues of non-compliance.
• Ensure consistent and ongoing communication with staff, referral sources, or community partners to benefit youth services.
• Develop, monitor, and implement recommendations for the youths Transition Plan.
• Complete assessment of participant needs.
• Coordinate safe housing placement for when youth discharge from the program.
• Assist youth in applying for resources.
• Ensure critical information is reported, documented, and regularly maintained.
• Plan and support transportation of clients as necessary.
• Attend program staff meetings and trainings required.
• Provide emotional support and model behaviors that will help lead youth to learn to positive life skills.
• Participate in rotating on call for participants to utilize in emergency or crisis situations.
