Job summary

Help of Southern Nevada seeks a dedicated youth case manager to provide direct services under the supervision of the Case Manager Supervisor. You will develop client-centered plans, track progress, and facilitate group sessions for homeless and at-risk youth in the Emergency Shelter Program.

Responsibilities include maintaining accurate records, coordinating identifications, and participating in multi-agency teams. Flexibility for varied hours and strong communication skills are essential.

Qualifications

• Bachelor degree in social work or four years of equivalent experience.
• Knowledge of local social service agencies and programs related to homeless/at-risk youth.
• Experience in case management, intake processes, and progress tracking.
• Ability to communicate across diverse backgrounds and maintain boundaries.
• Knowledge of applicable statutes, codes, and agency policies.
• CPR certification and TB test as required.
• Must pass background and pre-employment drug screening, and have NV driver's license.

Responsibilities

• Create client-centered case plans within 72 hours of intake.
• Monitor progress and facilitate weekly sessions addressing goals.
• Complete monthly case plan reviews with client signatures.
• Enter case notes daily into HMIS reflecting progress.
• Assist youth in obtaining vital identification documents.
• Maintain audit-ready files and respond to audits.
• Provide crisis intervention as needed.
• Attend weekly supervision meetings and report incidents.
