The position emphasizes coordinating with families, documenting in EHR, and maintaining strong community agency relationships to ensure quality care.

Qualifications

• High School Diploma with 5+ years’ experience delivering case management and linkage services to youth and families.
• B.A./B.S. in mental health field and experience with SED children in care settings.
• 2 years of college + 3 years experience with SED children in hospital/group home/day treatment/classroom.

Responsibilities

• Comply with policy and procedures including DMH Manual and LAUSD requirements.
• Participate in Child and Family Treatment (CFT) or School Meetings.
• Provide services in community, home, telehealth, or preferred meeting location.
• Link clients with services and follow up to ensure needs are met.
• Assist clients with obtaining California ID and immigration documentation as needed.
• Provide rehabilitation services to improve daily living and social skills.
• Collaborate with multidisciplinary treatment team and document in EHR.
• Maintain liaison with community agencies to foster linkages.
• Complete progress notes within 24 hours and ensure proper billing documentation.
• Attend trainings and weekly supervision; on-call during crises.

Education

High School Diploma with 5+ years’ experience delivering Case Management

B.A./B.S. in mental health field

2 years of college + 3 years experience with S.E.D. children

Ability to work with multidisciplinary teams

Bilingual Spanish preferred

Strong written communication
