Job summary

Urban Street Angels, Inc. in La Mesa, CA is seeking a Care Coordinator to manage a caseload of Transitional Age Youth, ensuring engagement in services and progression through the program’s phases to housing stability.

The role emphasizes coordination across teams, trauma-informed care, and precise documentation in HMIS/ETO, with duties from intake to program exit and ongoing support for education and employment goals.

Qualifications

• Bachelor's degree (preferred) in social work, human services, psychology or related field; equivalent work experience up to one year.
• Experience working with transitional age youth or individuals experiencing homelessness.
• Excellent computer skills; proficiency with Google Workspace and electronic case management systems.
• Strong interpersonal and communication skills, both written and verbal.
• Demonstrated knowledge of trauma-informed care, harm-reduction advocacy, and support techniques.
• Ability to work independently, manage time effectively, and prioritize tasks.
• Cultural sensitivity and experience working with diverse populations.

Responsibilities

• Serve as the primary point of contact for participants from intake through program exit.
• Complete intakes, enrollments in HMIS/ETO and/or other internal systems, and initial assessments to identify needs and eligibility.
• Develop and oversee individualized case plans aligned with the program’s phased model.
• Coordinate care with CSS, OT staff, Housing Navigators, Behavioral Health, and community resources.
• Manage a caseload of 45–50 participants and monitor engagement across all phases.
• Collaborate with CSS, OT staff, and Housing Navigators to ensure referrals are completed.
• Coordinate with housing staff to support housing readiness and transition.
• Maintain accurate, timely documentation in HMIS and other systems.
