Zusammenfassung

Sunlight Health in Santa Maria, CA seeks a Lead Care Manager to work with youth and families involved in child welfare and justice systems. You will coordinate care across providers, conduct assessments, and develop care plans with youth, with emphasis on lived experience and strong documentation.

Flexible 40-hour week, local driving required, bilingual in English/Spanish valued. You will join a team partnering with CASA and ECM programs to support reunification and health outcomes.

Qualifikationen

• Bachelor's degree in social work, public health, psychology, human services, or related field OR two+ years of relevant experience
• 1+ years of experience as Lead Care Manager or with youth/families in child welfare, foster care, probation, or juvenile justice
• CPR certification (or willingness to get it) and ability to pass a background check
• Bilingual English/Spanish is highly valued

Aufgaben

• Reach and engage eligible youth and caregivers, working with CASA volunteers and Advocate Supervisors
• Meet youth where they are (homes, placements, schools, juvenile hall, courthouse)
• Complete whole-person assessments and write care plans
• Coordinate care across doctors, behavioral health providers, social workers, probation, attorneys, schools, and caregivers
• Support youth through transitions, such as hospital discharge or placement changes
• Help youth keep Medi-Cal/health plan coverage and arrange transportation
• Join Child and Family Team, IEP and 504 meetings, attend court hearings as needed
• Maintain clear, timely records (visits within 72 hours) and meet monthly contact and health plan requirements
• Carry a caseload of up to 50 youth
