You will coordinate person-centered services with local systems and collaborate across schools, courts, and agencies to improve continuity of care. This full-time role emphasizes confidentiality, teamwork, and efficient documentation, supporting crisis response and community-based care for youth

Qualifications

• Bachelor's degree in a human services field or equivalent experience.
• Strong communication skills, both verbal and written.
• Ability to collaborate with diverse partners and maintain confidentiality.
• Knowledge of trauma-informed approaches and community resources.

Responsabilités

• Connect youth and families with behavioral health and crisis resources through community partners.
• Coordinate person-centered services with local systems of care for youth with frequent system contact.
• Build and maintain relationships with schools, courts, Children’s Division, families, and service providers.
• Collaborate on a multidisciplinary team to improve care continuity and access.
• Participate in Crisis Intervention Teams and trainings; attend calls and meetings.
• Complete required documentation accurately within deadlines.
