Job summary

Youth Opportunity is hiring a Case Manager at the Gallatin Primary Assessment Center to coordinate assessment services, documentation, and transition planning under clinical supervision. Responsibilities

  • Provide case management services and maintain ongoing communication with youth, families, agency representatives, and treatment team.
  • Coordinate or arrange required mental health and substance use screenings and assessments.
  • Complete performance summaries, treatment plan reviews, transition plans, aftercare plans, and discharge summaries.
  • Maintain accurate, timely, and objective case records according to organizational and contractual requirements.
  • Serve as a liaison among the program, Tennessee Department of Children’s Services, outside agencies, and youth families.
  • Prepare youth and families for program visits, family conferences, transition planning, and aftercare responsibilities.
  • Connect youth and families with community resources.
  • Assist with post-release data collection as required by policy and Medicaid guidelines.
  • Participate in treatment team meetings and communicate progress, needs, and placement recommendations.
  • Collaborate with leadership on performance-improvement activities.
  • Conduct periodic case-record reviews to support Youth Opportunity policies.

Location: Gallatin, TN
Job Type: Full-Time
Pay: $48,000 - $52,500 per year, based on experience

About Youth Opportunity & Gallatin Primary Assessment Center

Youth Opportunity is a nationally recognized leader in residential and behavioral health treatment for at-risk youth. Gallatin Primary Assessment Center in Gallatin, Tennessee is a secure short‑term facility serving boys ages13 to17 who require assessment and stabilization. The center offers continuous observation, clinical evaluation, mental health care, and life skills programming during initial placement.

Youth typically remain for about 30days, receiving individual and group therapy, educational services, victim-impact awareness, and family engagement. Small group living units support informed placement decisions and safe transition to longer‑term residential or community settings.

Position Summary – Case Manager Job in Gallatin, TN

We are hiring a Case Manager to coordinate assessment services, documentation, communication, and transition planning at Gallatin Primary Assessment Center. Working under the clinical supervision of a licensed mental health professional, you will serve as an important liaison among youth, families, the Tennessee Department of Children’s Services, the treatment team, and other community partners.

Case Manager Responsibilities

Other Duties Include
  • Maintain a high standard of role-model behavior for youth
  • Participate in required training and professional development activities
  • Report any acts, incidents, or conditions reflecting the possibility of inappropriate youth-to-youth or staff-to-youth relationships
  • Report any use of physical force and all unusual incidents according to established guidelines
  • Perform other duties as assigned or needed
Required Qualifications
  • Bachelor’s degree from an accredited college or university in counseling, social work, psychology, rehabilitation, special education, or a related human services field
  • Advanced verbal and written communication skills
  • Strong organizational, analytical, documentation, and time-management skills
  • Understanding of group leadership, group dynamics, and experiential education
  • Ability to work independently while collaborating effectively with youth, families, clinical professionals, and outside agencies
  • Ability to pass required background checks and pre-employment screenings
Preferred Experience
  • Two or more years of experience working with children or adolescents with serious emotional, behavioral, or substance use needs
  • Candidates with a master’s degree in an appropriate field may be considered without two years of professional experience
  • Experience working in a Medicaid provider environment
  • Experience with the Department of Children’s Services, juvenile justice agencies, residential programs, assessment centers, or other child welfare settings
  • Experience with case-record management and group-work techniques
Other Requirements
  • Physical Demands: Must be able to move throughout the property, participate in youth activities, and transport youth or staff when required. Must be able to meet applicable health requirements and successfully complete the PAR physical techniques evaluation and written certification examination.
  • Other: Must maintain an appropriate and valid state driver's license. Must meet all applicable state criminal background check and pre-employment screening requirements.
