Qualifications

Responsibilities

• Ensure educational, medical, emotional, and social needs of residents are met.
• Monitor residents' needs and arrange services as required.
• Provide case work services and monitor educational/vocational needs.
• Plan, coordinate, and document services to residents and their families.
• Coordinate discharge planning with family, regulatory agencies and case workers.
• On-call duties as scheduled.
• Coordinate and cooperate with internal and external contacts to ensure access to community resources.
