Job summary

Warren County ESC is seeking an Attendance Support Case Manager to work with Warren Co. Juvenile Court and Carlisle Schools. The role serves youth up to age 21, connecting them to services and monitoring school attendance.

You will collaborate with families, schools, and county agencies to support at-risk youth, provide home/community services, and track outcomes using an EHR system. A driver’s license and a bachelor’s degree are required.

Qualifications

Candidates must have a valid state driver’s license.
Bachelor’s degree in social services or related field.
Experience working with children/youth with behavior problems.
Ability to track outcomes through an electronic health record system.
Excellent communication skills.
Ohio license as a LSW or LPC is preferred but not mandatory.

Responsibilities

Assist families in strength based interventions.
Assist youth in preventing further contact with the court and/or other services.
Provide services in the home and/or community.
Assist in monitoring student attendance.
Assist in monitoring outcomes.
Interface with school districts and county agencies to assist with the needs of youth and families.
Coordinate resources such as food, clothing, school supplies and holiday programs as needed.
Assist in the prevention of issues that hinder the educational development of a youth.
Conduct CANS assessments (child, adolescent, strengths and Needs).
Assist in the mission of the Attendance Department.
