Job summary

Qualifications

• Ability to work in home, school, or community settings; bilingual preferred.

Responsibilities

• Deliver trauma-informed, culturally competent care to youth.
• Perform CANS Assessments and safety planning.
• Provide crisis intervention with weekly on-call rotation.
• Maintain up-to-date documentation and progress notes.
• Collaborate with schools, medical personnel, and providers.
• Support outreach and engagement with youth and families.
