Job summary

Fairview Hospital / Cleveland Clinic is seeking a trauma-informed, strengths-based case manager to support young adults who are experiencing or at risk of homelessness. You will manage a small caseload, conduct intakes and housing assessments, and develop personalized service plans to promote housing stability.

You will collaborate with landlords, community agencies, and internal teams to secure and maintain safe housing, while documenting progress and ensuring compliance with funder

Qualifications

• Experience or training in case management, housing support, and/or working with young adults.
• Strong assessment, planning, and documentation skills.
• Ability to build collaborative relationships with external partners.
• Commitment to trauma-informed care, harm reduction, and strengths-based practice.
• Excellent communication, organization, and client-focused problem-solving.
