Job summary

Professional Services Group in Jefferson County is seeking a Service Facilitator to provide intensive case management for individuals with severe emotional disturbance within the CCS Service Facilitation program.

The role collaborates with case managers, family advocates, mental health providers, school staff, and other community partners to develop and monitor multidisciplinary treatment plans; on-site in Jefferson, WI with mileage reimbursement.

Qualifications

• Must have reliable transportation daily, a valid driver’s license, and automobile insurance.
• Bachelor’s Degree in Social Work, Psychology, Criminal Justice, or other related fields.
• Minimum of one year of experience working with the severely emotionally disabled population.

Responsibilities

• Create individualized treatment plans for each assigned client.
• Develop a family support team comprised of formal and informal supports.
• Facilitate team meetings with service providers, family members, and family supports in which progress of goals and services are monitored.
• Develop a crisis/safety plan for the client and family.
• Exercise judgment to intervene as an advocate for clients in emergency and crisis situations.
• Attend client/family IEP meetings, psychiatry appointments, court proceedings, or other appointments that may require family support or advocacy.
• Safely transport clients as needed.
