Job summary

Professional Services Group is seeking a Service Facilitator in Jefferson County, WI. The role involves intensive case management for clients with severe emotional disturbance under the CCS Service Facilitation program.

You will collaborate with multiple providers to ensure comprehensive care, developing treatment plans and supporting families in their communities. The position is full-time, on-site in Jefferson with local travel and mileage reimbursement.

Qualifications

• Bachelor's degree in social services field required.
• Minimum of one year experience with severely emotionally disabled population.

Responsibilities

• Create individualized treatment plans for each client.
• Develop a family support team with formal and informal supports.
• Facilitate team meetings with providers, family members, and supports.
• Develop crisis/safety plan for client and family.
• Advocate for clients in emergency and crisis situations.
• Attend client/family IEP meetings, psychiatry appointments, court proceedings, or other appointments.
• Safely transport clients as needed.
