Job summary

Youth Advocate Programs, Inc. (YAP) is seeking a Part-Time Family Support Worker to partner with children and families to strengthen relationships and reach individualized goals.

Using YAP's Wraparound model, you will provide direct support, advocacy, and coordination of services in homes and community settings, helping families build on strengths and overcome challenges. Responsibilities include delivering ISP-driven services, coordinating interventions, maintaining a caseload of up to six

Qualifications

• Bachelor’s degree in a related field is required.
• 1+ year of experience with children, youth, and/or families in human services or related setting.
• Knowledge of strengths-based, family-centered, and trauma-informed practices preferred.
• Excellent verbal and written communication skills.
• Bilingual (Spanish/English) encouraged.
• Valid driver's license and reliable transportation required.

Responsibilities

• Provide direct services and supports to children and families per ISPs.
• Coordinate services and interventions while maintaining Wraparound fidelity.
• Implement ISP-identified strategies and interventions.
• Support families in achieving goals from individualized service planning.
• Maintain caseload of up to six families for intensive support.
• Document services per program requirements.
• Collaborate with Supervisors, Program Directors, families, and other providers.

Education

Bachelor’s degree in Social Work, Psychology, Human and Child Development, Counseling, Sociology, or a related field
