Job summary

Indiana Professional Management Group, Inc. is seeking a Wrap Facilitator to support Dubois County families. This full-time, hybrid role involves remote work from your home office and travel within the county for in-person meetings several times per week.

You will help families build wraparound teams, develop plans based on needs and strengths, and connect them with services to achieve goals, while participating in regular community-based team meetings.

Qualifications

• A BA/BS degree and at least 24 months of relevant experience in clinical, case management, skills building, child welfare, juvenile justice or K-12 education

Responsibilities

• Create a wraparound plan based on the strengths and needs of the family
• Help families access services and supports they need to meet their goals

Let's talk about the Wrap Facilitator position!

This full-time, hybrid position combines remote work from your home office with travel of up to an hour and a half (1.5 hours) for community-based meetings several times a week. This position serves Dubois County, and to accommodate required in-person meetings, applicants must reside within Dubois County, Indiana. Candidates must be able to travel throughout the county as needed to attend community-based meetings.

What You'll Do:

• Help families develop their wraparound team made up of caring professionals, friends, and family
• Create a wraparound plan based on the strengths and needs of the family
• Help families access services and supports they need to meet their goals
• Facilitate regular community-based wraparound team meetings to review plan of care

Who You'll Work With:

• Wraparound facilitators work with emotionally and/or behaviorally challenged children and their families

Requirements

• A valid driver's license, car insurance, and reliable transportation
• The ability to pass thorough background screens
• A BA/BS degree and at least 24 months of one of the following experiences: clinical, case management, skills building, child welfare, juvenile justice or education in a K-12 setting
• Designated home office setting with computer, internet service, and smart phone
