Muskegon County is seeking a Wraparound Coordinator to support seriously emotionally disturbed (SED) children and their families. A Wraparound Coordinator, under general supervision, follows the fidelity of the Intensive Care Coordination with Wraparound model. A Wraparound Coordinator develops family-centered service plans for seriously emotionally disturbed (SED) children and their families. This employee will identify, link, coordinate, and monitor services with various community agencies to develop a community team to provide services to SED children. A Wraparound Coordinator will facilitate family-centered meetings with SED children and their families to gather information that will aid in the wraparound process. An employee in this class will manage and monitor individual wraparound budget plans, provide data for outcome review, and perform other work duties as assigned. The Wrapround Coordinator also performs Case Management II functions for individuals on their caseload who are not enrolled in Wraparound fidelity service provision.
An employee in this class generally performs sedentary work requiring the occasional lifting of objects weighing thirty-five (35) pounds or less.
A Wraparound Coordinator works in the HealthWest office, the community, and in the homes of their consumers. Use of county vehicle and/or personal vehicle will be required.

The purpose of this recruitment is to establish an eligible list from which to fill present and future vacancies in this classification in the County system.
