Choices-Coordinated-Care-Solution is hiring a Wrap Facilitator - Family Preservation to conduct strengths-based assessments and develop family-centered plans of care in a collaborative team environment. The role includes home visits, monitoring service delivery, and ensuring fiscal accountability.

Qualifications include a bachelor’s degree in social work or related field, two years of youth/family experience, and CANS certification within 45 days.

Qualifications

• Bachelor degree in social work or related field is required.
• Two years experience partnering with youth and/or families.
• Case management and team-building experience preferred.
• Experience developing and managing individual service delivery budgets is a plus.
• Bilingual skills (Spanish) a plus.
• CANS certification within 45 days of hire and thereafter.

Responsibilities

• Manages caseload within financial parameters.
• Uses flex funding to tailor services to child and family needs.
• Completes strengths-based assessments for plan of care with the team.
• Willingly performs other duties to advance Choices' mission.
