Job summary

Frederick Health is seeking a Wound and Skin Clinician to lead wound, ostomy, and continence care for both inpatient and outpatient clients. You will educate patients and families, implement evidence-based practices, and collaborate with the multidisciplinary team to optimize outcomes.

The role requires BSN, Maryland RN license, 3+ years of wound/ostomy experience, and certification in CWOCN/CWS/WCC or commitment to obtain within 2 years. Competitive pay and benefits are offered.

Qualifications

• Registered Nurse (Bachelor prepared) and Maryland license
• 3+ years adult nursing experience including wound, ostomy and/or incontinence care
• Active certification in wound specialty (CWOCN/CWS/WCC) or completion within 2 years of employment

Responsibilities

• Provide oversight of wound care to assist other healthcare providers in management of acute and chronic wounds
• Educate patients on wound care, infection prevention and injury prevention
• Provide and interpret evidence-based tools for staff nurses for decision support and best clinical practice
• Educate patient/family on ostomy care, pouching system management and signs of complications
• Treat stomal and peristomal skin complications
• Coordinate resources for patient supplies to reduce ED visits
• Provide post-discharge follow-up in outpatient settings
• Collaborate with surgeons, infection prevention, podiatrists and other team members
• Perform focused assessments to identify risks and care needs
• Establish care plans for wound/ostomy/continence healing
• Contribute to infection control and patient outcome improvements
• Advocate for reimbursement of supplies and access to long-term support
• Complete EMR documentation for WOC consults and evaluations
• Provide care to inpatient and outpatient clients
• Support departmental operations and quality initiatives

Job Summary

Supports, and is responsible for incorporating into job performance, the Frederick Health (FH) mission, vision, core values and customer service philosophy and adheres to the FH Compliance Program, including following all regulatory requirements and the FH Standards of Behavior. Wound and Skin Clinicians are experts in caring for patients with wounds, ostomies, and incontinence. Specifically, Wound and Skin Clinician nurses respond to the acute and rehabilitative needs of people with select gastrointestinal, genitourinary, and integumentary systems disorders. They provide direct care to people with abdominal stomas, wounds, fistulas, drains, pressure injuries, and/or continence disorders and can serve as educators, consultants, researchers, and administrators.

Example Of Essential Functions

• Provide oversight of wound care to assist other healthcare providers in management of acute and chronic wounds
• Educate patients on wound care, infection, and injury prevention
• Provide and interpret evidence-based tools for staff nurses for decision support and best clinical practice
• Educate the individual/family/caregiver on how to change, empty, and manage the pouching system; normal function of the ostomy; normal characteristics of the stoma and peristomal skin; and signs of complications to report to their healthcare provider
• Treat stomal and peristomal skin complications
• Navigate and suggest resources for patient supplies to reduce ED visits
• Provide follow-up care post-discharge in the outpatient clinic setting
• Collaborate with surgeons, infection prevention practitioners, podiatrists and other members of the interdisciplinary team
• Perform focused assessments including relevant history to identify risks, contributive factors, and reversible causes on incontinence
• Establish a plan of care for healing related to wound, ostomy and/or continence problem list
• Contribute value to their institution with regards to the effect of patient outcomes including, but not limited to: reducing instances of catheter-associated urinary tract infections (CAUTI), hospital-acquired pressure injury (HAPI), and improving urinary and bowel incontinence
• Advocate for reimbursement of supplies, services, and access to long-term support
• Completes comprehensive EMR patient documentation for WOC consults and/or scheduled appointment evaluations
• Provides services and care to both inpatient and outpatient customers
• Supports departmental operations and participates on various performance and quality initiatives
• Serves as a clinical resource/educator or preceptor for staff learning principles of wound care

Minimum Education, Training, And Experience Required

• Registered Nurse (Bachelor prepared) and licensed in the state of Maryland
• Masters preferred and encouraged
• Minimum of three years adult nursing experience that includes caring for patients with wound, ostomies and/or incontinence problems
• Active certification in a nursing wound specialty i.e. CWOCN (preferred), CWS, WCC
• Or completion of wound certification within 2 years of employment, to include salary adjustment once achieved
• CPR training (AHA) for Basic Life Support for Healthcare Provider

Patient Contact

Must demonstrate and maintain current knowledge and skills in providing appropriate care/contact for patients in the following age groups:

• Neonate (0 thru 30 days)
• Infant (31 thru 12 months)
• Child (13 months thru 12 years)
• Adolescent (13 years thru 17 years)
• Adult (18 years thru 65 years)
• Geriatric (66+ years)

Physical Demands

Medium-Heavy Work - Lifting up to 75 pounds on an infrequent basis (less than one lift every three minutes) and/or carrying up to 35 pounds while walking. Pushing or pulling up to 115 pounds infrequently or greater than 115 pounds occasionally (no more than once per hour).

Pay is based on experience, skills and education. If position is part-time, salary will be pro-rated based on scheduled hours. The pay range may also vary within the stated range based on specialty if applicable. Non-Exempt positions may have shift differential and/or Overtime paid, if applicable.

Pay range: $33.64/hr Min - $52.77/hr Max
