Qualifications

• 0–6 months related nursing experience.
• Working knowledge of disease state management, bio-psychosocial model, and clinical pathways.

Responsibilities

• Uses the nursing process to assess, plan, implement and evaluate care.
• Documents steps of the nursing process with medical terminology.
• Coordinate with other disciplines and patients, families and caregivers.
• Facilitate patient discharge with referrals and education resources.
