Job summary

Qualifications

• Knowledge of disease state management, bio-psychosocial model, and clinical pathways

Responsibilities

• Use the nursing process to assess, plan, implement and evaluate patient care.
• Assess and organize patient care considering age, culture, and psychosocial factors.
• Document steps of the nursing process using medical terminology.
• Address problems and progress toward goals and facilitate patient discharge with referrals and education.
• Collaborate with other disciplines; participate in PI and educational activities.
