Qualifications

• As of 4/27/2025, BSN required for the role, per update to job description.

Responsibilities

• Assess physical, social, and psychological status and information from interdisciplinary team to inform care.
• Plan and develop individualized care plans with patient, family, and team involvement.
• Implement care by coordinating with the interprofessional team and allocating time, supplies, and resources.
• Evaluate patient responses and adjust the care plan, ensuring timely documentation and compliance with licensure.
