MFM Health is seeking a Wound Nurse to join our clinical team in Massachusetts. This in-person per diem role focuses on assessing symptoms, triage, wound care, and coordination of patient care.

The ideal candidate will have an RN or LPN license (MA), at least five years of nursing experience (home health or wound care preferred), and WOCN certification. Strong communication and teamwork are essential for success in this role.

Qualifications

  • Current RN or LPN license (as required by MA state regulations)
  • Minimum of five years clinical nursing experience; home health or wound care experience strongly preferred
  • WOCN certification (Wound, Ostomy, and Continence Nursing) required
  • Excellent telephone communication skills
  • Ability to make sound clinical decisions quickly and accurately
  • Strong organizational and documentation skills
  • Ability to manage stress, multitask, and remain calm in urgent or emotionally charged situations
  • Team-oriented with a proactive, collaborative approach to care, and clear communication skills

Responsibilities

  • Evaluate and assess patients’ symptoms during in-person encounters.
  • Deliver consultation and direct care for wounds, fistulas, ulcers, and skin issues.
  • Educate patients and healthcare staff on wound and skin care practices.
  • Clearly document all interactions and care plans in the medical record.
  • Work with other departments to meet patient needs and support team
  • Actively participate in team communication and cross-functional support.

Wound Nurse

Job Type: Per-Diem | In-Person Only

Overview

We are seeking a highly skilled and compassionate Wound Nurse to join our clinical team. This is an in-person per diem position responsible for assessing and evaluating patients’ reported symptoms, determining the most appropriate course of action, and ensuring comprehensive coordinated care. The ideal candidate is a team player with strong communication skills, clinical expertise, and the ability to handle a variety of situations with professionalism and empathy.

Key Responsibilities
  • Patient Assessment & Triage:
    • Evaluate and assess patients’ reported symptoms during in-person encounters.
    • Administer in-person clinical triage and basic patient care as needed.
  • Care Coordination:
    • Manage Complex Wounds, Fistulas, and Skin Conditions - Deliver consultation and direct care for patients with wounds, fistulas, pressure injuries, skin tears, ulcers, and other skin integrity issues, including developing and evaluating treatment plans.
    • Serve as a Clinical Resource and Consultant - Provide expert guidance to nurses and healthcare staff on wound and skin care management.
    • Educate Patients and Healthcare Staff - Conduct formal and informal education, staff training, in-services, and updates on best practices, products, and advances in wound and skin care.
  • Communication:
    • Clearly and accurately document all interactions and care plans in the medical record.
  • Teamwork & Support:
    • Work closely with other departments and assist clinical staff to meet the needs of the patient.
    • Actively participate in team communication and cross-functional support.
    • Serve as a resource by identifying teaching moments and constructively advising peers.
  • Professional Conduct:
    • Maintain a professional demeanor at all times.
    • Report concerns or critical observations to appropriate team leads or supervisors.
    • Adhere to organizational policies and demonstrate commitment to high-quality patient care.
