Wound Care/Treatment Nurse

Position and Schedule

  • Position:Wound Care/ Treatment Nurse (LVN or RN)
  • FLSA Status:Exempt (Salary)
  • Job Status:Full-Time
  • Schedule:Monday through Friday (8:30AM until 4:30PM)
    • On-call and weekends as needed

Responsibilities

  • Identify, manage, and treat specific skin conditions and primary and secondary lesions, such as skin abrasions, foot problems such as corns and callouses, pressure injuries/ulcers, bacterial, parasitic and viral skin infections, scalingpapular diseases, and benign tumors.
  • Provide assessment and diagnostic services to residents. Perform skin assessments using techniques including observation, inspection, and palpation.
  • Provide therapeutic services; confer with the Care Planning Team in the development of the care plan.
  • Using established protocols under the supervision of the physician, provide treatment for uncomplicateddermatologic disorders as directed by the Medical Director or attending physician.
  • Ensure that residents with pressure injuries/ulcers receive appropriate prophylaxis and treatment, such as daily inspection, turning and activity, well-planned diet, and maintaining a clean, dry bed.
  • Assist in the admission/readmission assessments and discharge planning of residents.
  • Work with other health agencies and family members in the physical and emotional transitions of admission, care, and discharge of residents, as necessary.
  • Provide information to resident and family members of prevention techniques and other ways to maintain or improve the general skin health of the resident.
  • Report known or suspected incidents of fraud to the Administrator.
  • Participated as requested by the Medical Director or Director of Nursing in the planning and conducting of in-service training classes concerning skin care & prevention skin issues, reporting skin concerns, completion ofPCC or other paper forms required for skin process and nursing skills and procedures used within the facility.
  • Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home.
  • Serve on the Care Plan Team; work with the Care Plan Team in development comprehensive assessment and care plan for assigned residents.
  • Serve on, participate in, and attend various other committees/meetings of the Community, as required and as appointed by the Administrator. Such as but not limited to: Standards of Care, Morning Clinical meetings,QAPI, etc.
  • Provide timelyPCC (Point Click Care) E.H.R documentation, required written and oral status reports of residents being treated. Make written and oral reports/recommendations to the attending physician, Medical Director, and the Director of Nursing concerning the status and care of residents.
  • Evaluate and implement recommendations from established committees as they pertain to the medical activities of the Community and the skin care of residents.
  • Other duties, as assigned and deemed appropriate by the Director of Nursing or Community Administrator.

Minimum Qualifications

  • Current and valid unencumbered LVN or RN license to practice as a nurse in the state of Texas. (Required)
  • Minimum of 2 years experience as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN). (Required)
  • One year of experience in a nursing supervisory capacity and previous experience in Wound Care. (Preferred)
  • Experience in a Skilled Nursing environment. (Preferred)
  • Certified Wound Care/Treatment Nurse. (Preferred) We will pay for you to get CERTIFIED!
